I have been using the native Reminders new task shortcut because it offers the ability to be prompted whether I need an alert or not. I tried using the GoodTask shortcut for creating a task, but it seems to only offer the ability to require an alert date every time (not giving you the option each time). Am I doing something wrong? I want to be able to input "Yes" or "No" for whether I need a date/time or not, and if "Yes," then it would show the date input screen. Any help is appreciated!

Since GoodTask's Shortcut action provides the options to set variables, you can make a shortcut to ask if you want an alert before the main GoodTask action.
